What it is

Oxygen uptake, strength, and balance as risk markers.

Your maximal oxygen uptake, VO₂max, is a measure of how much oxygen your body can take up and use when you exert yourself to the fullest. It says something about the heart, lungs, blood, and muscles all at once, and it is one of the markers most strongly linked to how long you live and how many of those years you are healthy. Despite that, it is rarely measured in routine healthcare.

The lactate threshold shows where in your heart rate range your body goes from sustainable to exhausting. With it measured, you know which heart rate levels to hold to improve and maintain your VO₂max, and where your zone 2 lies: the easy endurance training that does the most for heart, vessels, and brain over time.

Grip strength is measured because it is a strong indicator of future health: in large population studies, low grip strength is associated with a higher risk of cardiovascular disease and premature death, independent of other risk factors. It is simple to measure and can be influenced with training.

The purpose is medical. Low fitness, low muscle strength, and impaired balance are risk markers for cardiovascular disease, muscle loss, and falls, and all three can be influenced. The tests are chosen based on your health picture, the pre-assessment is done according to protocol, and the result is interpreted clinically and turned into actions and follow-up.

This assessment is the Health Assessment plus a test day where your physical function is measured. Oxygen uptake and lactate threshold on an ergometer, or a submaximal fitness test on a bike if you cannot complete a full VO₂max test. Grip strength, balance, sit-to-rise, mobility. The tests are carried out by Aktivitus according to a protocol I have designed, and the test order is fixed so that exhausting elements come after the main test and the measurements are reliable.

Then I interpret all of it together with your blood markers, analyzed from a functional medicine perspective, and your history. You receive a health plan based on your measured values, with numbers to follow over time.

We prepare everything, you choose the time. The booking link to Aktivitus comes from us in the confirmation, and you choose the day that suits you. Aktivitus carries out the tests in Stockholm, Göteborg, Malmö, or Linköping.

A measured VO₂max compared with the age- and sex-specific norm, instead of an estimate from a watch. A baseline to improve, and a number to compare against in two years.

The lactate threshold shows where your zones lie. Many train too hard in the easy session and too easy in the hard one. With the threshold measured, you know where the limits are for you specifically.

You are middle-aged and want to preserve function

Strength, balance, mobility, and muscle power are independent predictors of how independent you are in twenty years. Measured now, they can be influenced in time.

Oxygen uptake · VO₂max and lactate threshold on an ergometer, with your training zones as the result. Strength · grip strength in both hands against the EWGSOP2 threshold. Balance · single-leg stance with eyes closed against the age norm. Functional mobility · sit-to-rise test, 0 to 10 points. Mobility · shoulder, ankle dorsiflexion, and forward bend, including side-to-side differences. Submaximal fitness test · on a bike, for those who cannot complete a full VO₂max test. Muscle power · vertical jump (optional). Endurance · plank, wall sit, and chair stands.

Each marker is interpreted against published reference values, not against a general scale. Where the evidence is weaker, I say so: the plank is a fitness reference, not a mortality-validated marker, and it is treated accordingly.

Practical

How it works.

  1. You book and receive instructions for the blood draw and health questionnaires, plus the booking link to Aktivitus.
  2. Blood draw at Synlab, fasting in the morning.
  3. Test day at Aktivitus. Come rested: avoid hard training for 24 hours before, standardize caffeine and meals, avoid alcohol the day before. Bring training clothes.
  4. I analyze all the material before we meet.
  5. Video visit, 60 minutes.
  6. The health plan follows in writing afterwards.

The video visit takes place wherever in Sweden you live.

Frequently asked questions.

Do I need to be fit to do the test?

No. The test is designed to measure where you stand, not to make you perform. Before the test day, a pre-assessment is done according to the protocol, with health questionnaires and a check for contraindications, and the ergometer test is carried out in stages with medical stopping criteria: it is stopped when you cannot go on or when the protocol’s limits are reached. Being untrained is no obstacle. The test then gives you a starting value to work from.

How does it differ from a standard fitness test?

A standard fitness test gives you a VO₂max value. Here that value is put in context with your blood markers, your strength, your balance, your mobility, and your history, and becomes a plan.

Is body composition included?

Not in the test day. You follow your body composition according to a standardized instruction you receive from me, so that the measurement can be repeated the same way over time. If you want an MRI-based measurement, AMRA is available as an add-on.

Can I repeat the test?

Yes. The follow-up is an important part of the value. Book the follow-up at the same time of day for comparability. At follow-up you have a baseline, which makes the analysis considerably more accurate. A repeat test day costs 5,900 SEK including my interpretation against your previous values. We go through the result at a check-in if you are a member, otherwise at a 60-minute follow-up visit for 6,900 SEK. In Personal physician, the test day is included every year.

What happens if something unexpected is found?

I am a licensed physician and specialist. If something is found that needs further investigation, I write a referral when it is medically warranted. The tests are carried out under controlled conditions according to a protocol with rules for when testing is advised against.
